Food Service Forms
The Â鶹AV Unified School Â鶹AV participates in the National School Lunch and/or School Breakfast Programs (NSLP). New NSLP applications now known as School Funding Forms are now being accepted at your school cafeteria, in the school office, or at the Â鶹AV Office. You are encouraged to complete a form to help support the school's funding. Below you will find a link for the form. Remember this form is for is for more than a meal!
NSLP meals are served every school day at participating schools. A new form is required every school year (one per household). You or your children do not have to be U.S. citizens to qualify for free or reduced-price meals. If you need additional information please do not hesitate to contact us at (916) 375-7604 ext. 1060.
